Investigation Summary
The accident occurred approximately 40 minutes after sunset in visual meteorological conditions. The Falcon 50 (N784B) was arriving from Morristown on an IFR flight plan, while the Piper Archer (N1977H) was operating VFR without a flight plan. The Teterboro local controller was managing high traffic volume, including six aircraft simultaneously. Although both aircraft were in radio contact with the tower, the controller provided the Falcon crew with an erroneous traffic advisory, describing the Piper as being at their one o'clock position when it was actually closer to their twelve o'clock and obscured by the jet's nose or the horizon. The Piper pilot also provided inaccurate position reports, stating he was 10 miles west when he was actually 5 miles west, and later 1 mile west when he was 2.5 miles west. These inaccuracies, combined with the lack of radar in the Teterboro tower at the time, prevented the controller from recognizing the imminent collision. The Falcon's left wing struck the right side of the Piper's engine compartment, causing both aircraft to fall into a densely populated neighborhood.
Final Conclusions
The National Transportation Safety Board determined the probable cause was a breakdown in air traffic control coordination which resulted in an air traffic conflict and the inability of the DA50 flightcrew to see and avoid the other aircraft. Contributing factors included an erroneous and inadequate traffic advisory from ATC and the physiological limitations of human vision and reaction time at night. ATC management was also cited for failing to ensure controllers followed prescribed procedures and for failing to correct operational deficiencies at the Teterboro facility.
