Aeroméxico Flight 498, a DC-9, was on approach to Los Angeles International Airport when it collided mid-air with a private Piper PA-28 Archer II over Cerritos. The Piper had inadvertently entered the Los Angeles Terminal Control Area (TCA) without clearance and was not equipped with a Mode C transponder. The collision sheared off the DC-9's horizontal stabilizer, causing it to plunge into a residential neighborhood. All 67 people on both aircraft and 15 people on the ground were killed.

Investigation Summary

The accident occurred in the highly congested airspace of the Los Angeles Terminal Control Area (TCA). The Piper PA-28, piloted by William Kramer, was flying under Visual Flight Rules (VFR) and entered the restricted TCA without communicating with Air Traffic Control. At the same time, the Aeroméxico DC-9 was being vectored for an ILS approach to Runway 25L. The air traffic controller was distracted by another unauthorized aircraft (a Grumman Tiger) that had also entered the TCA, which diverted his attention from the impending conflict between Flight 498 and the Piper. Because the Piper lacked a Mode C transponder, the controller's radar display did not show its altitude, and the automated conflict alert system failed to trigger. The two aircraft collided at approximately 6,560 feet, with the Piper's engine and propeller slicing through the DC-9's left horizontal stabilizer.

Final Conclusions

The NTSB determined the probable cause was the limitations of the ATC system to provide collision protection, including both procedures and automated redundancy. Contributing factors included the unauthorized entry of the Piper into the TCA and the inherent limitations of the 'see and avoid' concept in high-density airspace.
