Investigation Summary
The accident occurred during large-scale air defense exercises involving the Ukrainian military at Cape Opuk in Crimea. A 5V28 missile was launched from an S-200V battery at a target drone. However, the missile's radar seeker bypassed the drone and instead locked onto the Tu-154M, which was flying along international airway B-145. A contributing factor was that the exercise area was only restricted up to an altitude of 8,000 meters, while the airliner was cruising at 11,100 meters, leading military operators to believe the airspace above the restriction was clear. Because the S-200 is a long-range system designed to track large targets, the airliner provided a much stronger radar return than the drone. The missile traveled approximately 240 kilometers before detonating. Initial Russian reactions suspected terrorism due to the proximity to the September 11 attacks, but forensic evidence, including shrapnel holes in the recovered fuselage and radar data, quickly pointed to a missile strike.

Ukraine eventually accepted political responsibility for the accidental shootdown and paid ex-gratia compensation to the families of the victims.
