Investigation Summary
The investigation revealed a catastrophic breakdown in the 'safety net' systems. At the time of the accident, the Skyguide Area Control Center in Zurich was understaffed, with only one controller, Peter Nielsen, managing two frequencies. Crucial safety systems, including the Short Term Conflict Alert (STCA) and the main telephone lines, were either disabled or degraded for maintenance. Nielsen only noticed the conflict less than a minute before impact. He instructed the Tu-154 to descend, unaware that the DHL 757's TCAS had already commanded it to descend as well.
