Caspian Airlines Flight 7908 crashed into an open field near Jannatabad, Iran, after suffering a catastrophic uncontained engine failure in its number one (left) engine. The failure led to a fire and the destruction of critical flight control systems, causing the aircraft to enter a steep, high-speed descent. All 168 passengers and crew members were killed when the aircraft impacted the ground at high velocity, creating a 10-meter-deep crater.

Investigation Summary

Approximately 16 minutes after departing Tehran, while climbing through 28,800 feet, the first-stage rotor of the low-pressure compressor in the number one engine disintegrated due to fatigue failure. The resulting uncontained failure sent high-velocity debris through the tail section of the aircraft. These fragments severed the number one and number three hydraulic systems and partially cut the fuel lines to the number two engine. The leaking fuel and hydraulic fluid were ignited by hot engine components, resulting in a massive fire in the tail. This intense fire led to the thermal destruction of the mechanical actuator rods responsible for controlling the rudder and elevators. With the loss of both hydraulic power and the mechanical linkages to the primary control surfaces, the crew was unable to maintain level flight. The aircraft pitched down into a near-vertical dive, impacting the ground at a speed estimated to be around 500 knots.

Final Conclusions

The final investigation report, released by Iranian authorities with assistance from the Russian Interstate Aviation Committee (IAC), concluded that the primary cause was the fatigue failure of the low-pressure compressor rotor disc in engine No. 1. The disintegration of this disc caused a chain reaction of mechanical and structural failures, including the loss of all elevator and rudder control and a severe in-flight fire. In 2016, the Tehran prosecutor's office further attributed blame to the manufacturer and maintenance personnel for a faulty compressor valve and issues regarding the aircraft's acquisition and operation.
