Three Bristol Blenheim IV aircraft of 15 Squadron, South African Air Force, performed forced landings in the Libyan desert after becoming lost during a familiarization flight from Kufra Oasis. Investigation Summary

On May 4, 1942, three Blenheims (Z7513, Z7610, and T2252) took off for a square-pattern familiarization flight. A critical error occurred when the leader, Major de Wet, refused a weather balloon check; unknown to the crew, winds were significantly different than predicted, blowing them off course. When Kufra attempted to provide a bearing, the crew misinterpreted the radio signal '3-0-5' as a heading, leading them further into the desert. After fuel exhaustion, all three aircraft landed successfully. The crews of Z7610 and T2252 landed together, while Z7513 landed separately. A severe sandstorm then grounded search efforts for several days. In their desperation, the men consumed poisonous compass alcohol and sprayed themselves with fire extinguisher fluid to cool down, causing severe skin injuries. When rescuers finally arrived, they found that several men, including Major de Wet, had ended their own lives with service revolvers to escape the agony of thirst. Air Mechanic Juul, found with the two grouped aircraft, was the sole survivor.
