Investigation Summary
The aircraft involved, RA-85164, had exhibited a persistent tendency to roll to the left since its last major maintenance in 1991. To compensate for this during Flight 3949, the crew configured the fuel system to feed engines only from the left wing tanks, intending to lighten the left side and level the wings. However, they overcompensated. As the aircraft cruised at 10,600 meters on autopilot, the resulting right-side weight bias caused the autopilot to reach its compensation limit. The aircraft then entered a right-hand bank that went unnoticed by the crew until it reached a critical angle. The Tu-154 entered a steep spiral dive, reaching a vertical speed of 300 m/s. While the aerodynamic loads were extreme, the aircraft remained largely intact until it impacted the mountain at a 70-degree pitch angle, resulting in total disintegration.

The Interstate Aviation Committee (IAC) concluded the disaster resulted from a combination of factors: a pre-existing mechanical roll tendency, improper fuel management by the crew to correct said tendency, and a lack of adequate monitoring of flight instruments while the autopilot was engaged. The design of the Tu-154's warning systems was also cited for failing to alert the crew to the developing extreme bank angle in a timely manner.
