Investigation Summary
The accident occurred due to a combination of pilot error and non-standard air traffic control communication. The Piper Aztec, G-AYDE, was instructed to taxi to 'Alpha 26', a non-standard phrase that omitted the word 'holding point'. The pilot of the Aztec, possibly confused by the phrasing or lacking situational awareness, continued past the holding point and onto the active runway. At the same time, Court Line Flight 95 was at high speed on its takeoff roll. The Aztec's passenger, an experienced pilot, noticed the incursion and questioned the pilot, but received no response before the collision. The One-Eleven's port wing struck the Aztec, causing a massive fuel leak and destroying the smaller aircraft's cabin. The subsequent evacuation of the One-Eleven, which carried 91 of the 93 total souls involved in the accident, was hampered by jammed forward doors, later attributed to incorrectly stowed escape slides and an incorrect part fitted to one door mechanism.
