Investigation Summary

The flight departed Santa Ana at 09:07 PST under VFR. The aircraft, a Sikorsky S-76B manufactured in 1991 (MSN 760379), was carrying eight passengers and one pilot. As the helicopter progressed toward its destination, the pilot encountered a low cloud ceiling and haze. Due to traffic at Burbank Airport, the helicopter held over Glendale for 12 minutes before being cleared to proceed. The pilot requested and received a Special VFR clearance to transit the controlled airspace. As the terrain rose near Calabasas, the gap between the ground and the cloud ceiling narrowed significantly. At 09:44, the pilot informed air traffic control that he was climbing to 4,000 feet to get above the layers. During this climb, the helicopter entered the clouds, depriving the pilot of a visual horizon. The NTSB determined that the pilot likely suffered from 'the leans,' a form of spatial disorientation where the inner ear perceives the aircraft is level or turning in the opposite direction. Consequently, while the pilot believed he was climbing, the aircraft entered a steep, banked descent. The helicopter struck a hillside at a high rate of descent, resulting in a post-crash fire and total destruction of the airframe.
