Investigation Summary

The investigation by the Accident Investigation Board Norway (AIBN) focused on the epicyclic module of the Main Gearbox. Metallurgical analysis revealed that a fatigue crack initiated from a micro-pit on the surface of a planet gear's bearing race. This crack propagated subsurface, a failure mode that bypassed the helicopter's chip detection system because it produced minimal metallic debris (spalling) before the final catastrophic fracture. The investigation also noted that the MGB had been involved in a road transport accident in 2015 where it fell off a truck, though it was inspected and returned to service by the manufacturer.
