Pan Am had been instructed to exit the runway at the third intersection (C-3), but the crew was confused by the 135-degree turn required and the lack of visibility, leading them to continue toward the fourth exit (C-4). Ambiguous radio communications, captain authority gradients, and a non-standard "we are at take off" transmission from the KLM First Officer set the stage for a collision that killed 583 people and reshaped global CRM training forever.
