Investigation Summary
The accident occurred during the 'Pierced Heart' (Il Cardiopalma) maneuver. The team split into two groups (five and four aircraft) to create the shape of a heart, while a tenth aircraft, the soloist, was intended to 'pierce' the heart by flying through the center toward the audience. The soloist, Lieutenant Colonel Ivo Nutarelli, arrived at the intersection point approximately 4-5 seconds too early and at a lower altitude than planned. His aircraft (Pony 10) struck the lead aircraft of the left-hand formation (Pony 1) and the lead of the right-hand formation (Pony 2). Pony 10 cartwheeled into the crowd, striking an ice cream truck and exploding. Pony 1 crashed onto the runway, and Pony 2 crashed into a taxiway, striking a parked U.S. Army UH-60 Blackhawk helicopter.

The primary cause was the soloist pilot's failure to maintain proper timing and altitude during the crossover maneuver. Contributing factors included the inherent risk of maneuvers directed toward the crowd and the lack of adequate safety buffers between the flight path and spectator areas. The emergency response was further complicated by differing medical standards and equipment between German and American first responders.
