Investigation Summary
The accident was initiated by the fatigue failure of a gas generator turbine blade in the left Walter M-601E engine just three seconds after the landing gear retraction command. The fracture occurred underneath the blade surface, rendering previous fluorescent penetrant inspections ineffective.
