A United States Air Force C-141B Starlifter and a German Luftwaffe Tupolev Tu-154M collided in mid-air at 35,000 feet over the Atlantic Ocean, approximately 65 nautical miles west of the Namibian coast. The collision occurred because the German aircraft was flying at an incorrect altitude for its direction of flight, in violation of the semicircular rule, while operating in an area with severely deficient air traffic control coordination. Both aircraft were destroyed and all 33 occupants were killed. Neither aircraft was equipped with a Traffic Collision Avoidance System (TCAS), which investigators concluded could have prevented the disaster.

Investigation Summary

The Luftwaffe Tu-154M (GAF 074) was en route from Cologne, Germany, to Cape Town, South Africa, to participate in a naval regatta celebrating the 75th anniversary of the South African Navy. The USAF C-141B (REACH 4201) had just departed Windhoek for Ascension Island, returning to the US after a humanitarian mission. The German crew had filed a flight plan requesting a climb to FL390 but remained at FL350, an altitude reserved for westbound traffic. The C-141B was legally cruising at FL350 on a northwesterly heading. Due to systemic failures in the African Aeronautical Fixed Telecommunications Network (AFTN), Windhoek ATC never received the flight plan for the German aircraft and was unaware of its presence. Luanda ATC, which was in contact with the Tu-154, failed to pass position data to Namibian controllers. The aircraft collided at a nearly right angle; the Tu-154 struck the lower fuselage of the C-141. A US reconnaissance satellite detected a bright flash at 15:10 GMT, marking the moment of impact. Search efforts were delayed for nearly 20 hours because communication breakdowns prevented Ascension Island ATC from confirming the C-141's departure status with Namibian authorities.
