Upon arrival at Tripoli, the crew encountered heavy fog with visibility reported between 100 and 800 feet, significantly below the required minima for the available NDB approach. Approximately one hour prior to the accident, a Soviet Aeroflot aircraft had diverted to Malta due to the same weather conditions. Despite these factors, the Korean Air crew continued their descent. The investigation highlighted significant human factors, including crew fatigue resulting from a lack of proper sleep, which impaired their mental concentration and performance. The Captain failed to conduct a proper weather briefing, and the First Officer and Flight Engineer failed to challenge the Captain's decision to descend below the decision height without visual contact with the runway environment.

The Libyan authorities, assisted by French specialists, concluded that the primary cause was the crew's decision to continue the approach in weather conditions below established minima. Contributing factors included crew fatigue, inadequate crew coordination (CRM), and a failure of the Ground Proximity Warning System (GPWS) to provide sustained warnings. The investigation also noted that the crew's simulator training was conducted on a DC-10 equipped with a different GPWS model than the accident aircraft. In December 1990, a Libyan court found the three flight deck crew members guilty of neglect, sentencing them to prison terms ranging from one to two years.
