Investigation Summary
The accident, often called the 'Windsor Incident,' exposed a critical vulnerability in the DC-10's wide-body design. Because the cargo doors opened outward to maximize internal space, they relied entirely on mechanical latches rather than internal pressure to stay closed. When the door failed, the rapid escape of air from the cargo hold created a massive pressure differential with the passenger cabin above. The cabin floor, not designed to withstand such loads, buckled downward. This collapse damaged the redundant control cables routed through the floor, leaving the pilots with only limited rudder and elevator control and partial engine power management. This exact failure mode would tragically recur two years later on Turkish Airlines Flight 981 with fatal results.
Final Conclusions
The NTSB determined the cause was a design deficiency in the cargo door latching mechanism which permitted the door to be closed and the handle stowed without the latches being fully engaged. A ramp agent had used physical force (his knee) to close the handle, which deformed a linkage, allowing the locking pins to bypass their intended slots while giving a false indication of security to the cockpit.
