Investigation Summary

During the approach to Islamabad, weather conditions were deteriorating with heavy rain and low visibility. The crew was cleared for a circling approach to Runway 12. The captain, however, descended below the Minimum Descent Altitude (MDA) and lost visual contact with the airfield. Instead of executing a standard missed approach, he attempted to fly a self-created approach using Place Bearing Distance (PBD) waypoints, which led the aircraft 4.3 nautical miles outside the protected airspace and directly toward the Margalla Hills. The investigation report described a toxic cockpit environment where the captain used a harsh, snobbish, and contrary tone, lecturing and belittling the first officer for over an hour. This behavior effectively silenced the first officer, who remained a passive bystander even as the aircraft headed for terrain. Despite 21 Enhanced Ground Proximity Warning System (EGPWS) 'PULL UP' alerts, the captain did not take corrective action until it was too late.
