Investigation Summary

The accident occurred after the crew accepted an offer from ATC for a straight-in approach to Runway 19. This required a rapid descent and quick reprogramming of the FMS. While attempting to select the 'Rozo' NDB, the pilot entered 'R' into the FMS. The system presented a list of beacons; the pilot selected the first one, 'Romeo', which was actually located near Bogotá, over 130 miles away. This caused the autopilot to initiate a sharp left turn toward the Andes mountains. The crew, focused on the descent and confused by the aircraft's movement, failed to recognize the navigational error until the Ground Proximity Warning System (GPWS) sounded. Although they attempted to climb, they failed to retract the speedbrakes, which significantly degraded the aircraft's climb performance.

The primary causes were the flight crew's failure to adequately plan and execute the approach, their lack of situational awareness regarding terrain, and the selection of an incorrect waypoint which led the aircraft off course.
