Investigation Summary

The collision occurred in the upper sector of the Zagreb FIR, one of the busiest air corridors in Europe at the time. The Inex-Adria DC-9 was climbing to Flight Level 350, while the British Airways Trident was level at FL330. Due to a shift change and high workload, the upper sector controller, Gradimir Tasić, was working alone. He instructed the DC-9 to maintain FL330 just as it reached that level, placing it on a direct collision course with the Trident. Crucially, Tasić communicated this instruction in Serbo-Croatian. This violated international protocol and deprived the British pilots of situational awareness. Seconds later, the aircraft collided at a relative speed of approximately 900 km/h.
