Investigation Summary
During the takeoff roll on Runway 13R, Flight 863 began an uncommanded rotation at approximately 80 knots, far below the calculated V2 speed of 124 knots. The aircraft's tail struck the runway 1,550 feet into the roll and continued to skid for 1,250 feet. Despite these clear indications of a flight control malfunction, the crew continued the takeoff. Once airborne, the aircraft pitched up violently. The First Officer reported he could not control the aircraft, and the stick-shaker activated almost immediately. The DC-8 reached a near-vertical attitude before rolling sharply and diving into the ground. Post-crash analysis revealed that a pointed, asphalt-covered object had become trapped between the leading edge of the right elevator and the horizontal stabilizer spar web access door. This object, likely blown into the gap by jet blast from a preceding aircraft during taxi or lineup, initially jammed the elevator in a 12-degree nose-up position. As aerodynamic loads increased and the crew applied forward pressure, the object scarred the metal and eventually punctured the spar web door, causing the elevator jam angle to decrease to an estimated 5 to 8 degrees nose-up. The airport was undergoing resurfacing at the time, and similar debris was found across the taxiways and aprons.

The NTSB determined the probable cause was a loss of pitch control caused by the entrapment of a foreign object in the right elevator assembly. A contributing factor was the crew's lack of responsiveness to the emergency, specifically the failure to monitor the takeoff adequately and reject the roll after the premature rotation and tailstrike occurred.
