Investigation Summary

The accident occurred during the final days of the Vietnam War as the United States began evacuating orphans. Shortly after takeoff, as the aircraft climbed through 23,000 feet, the locks on the rear loading ramp failed. The ramp blew open, causing an immediate explosive decompression that tore through the aft pressure bulkhead and severed the hydraulic lines and cables for the elevators and rudder. The flight crew, led by Captain Dennis Traynor, found the aircraft oscillating in a series of stalls and dives. By using the remaining functional ailerons and varying the power of the four engines, they managed to stabilize the aircraft and turn back toward Saigon. However, as they slowed for the final approach, the aircraft lost lift and descended rapidly, striking the ground, bouncing, and eventually breaking apart in a marshy area.
