On January 30, 2005, Hercules XV179 departed Baghdad at 16:24 local time for a routine low-level freight and passenger flight. Six minutes after takeoff, the crew reported a fire on board. The aircraft had been targeted by insurgents using ground fire, which penetrated the starboard wing fuel tank. Unlike many C-130s operated by the United States and Australia, the RAF Hercules fleet was not equipped with Explosion Suppressant Foam (ESF). The projectile caused the fuel-air mixture in the ullage (the space above the fuel) to ignite, resulting in an explosion that blew off the outer 23 feet of the wing, including the aileron. This rendered the aircraft uncontrollable. The crash site was located 25 miles northwest of Baghdad in a hostile area, which complicated the recovery and investigation. The Board of Inquiry later revealed that intelligence regarding previous surface-to-air attacks on American helicopters in the same area had not been shared with the Hercules crew, as the local intelligence officer was unaware of the flight's presence.
