Investigation Summary
The accident was rooted in a series of logistical failures at Paninternational's maintenance base in Düsseldorf. On September 5, 1971, mechanics draining residual fuel from a Boeing 707 used unlabeled 60-liter plastic canisters to collect approximately 100 liters of kerosene. These canisters, which were identical to those used for transporting demineralized water, were not marked and were subsequently left in a storage area. The canisters were then mistakenly loaded into the cargo hold of the accident aircraft, D-ALAR, in Düsseldorf to be transported. During the intermediate stop in Hamburg the following day, ground crew offloaded these canisters from the cargo hold, under the assumption they contained demineralized water, to fill the aircraft's common water-injection tank. Because the BAC One-Eleven 500 series utilized a single tank to feed both Rolls-Royce Spey engines, the contamination affected the entire propulsion system. When the pilots engaged the injection system during the takeoff roll to compensate for high temperatures and weight, the engines initially functioned normally while consuming the remaining pure water in the lines. As the aircraft reached a height of approximately 700 feet (213 meters), the kerosene reached the combustion chambers, causing an uncontrolled temperature spike that melted turbine blades and led to a total loss of thrust in both engines.
Final Conclusions
The investigation concluded that the primary cause was the accidental filling of the engine water-injection system with kerosene. This was facilitated by the use of unlabeled, identical transport containers for both fuel drainage and water storage. The resulting dual engine failure at low altitude left the crew with no choice but to attempt a dead-stick landing on the Bundesautobahn 7. While the initial touchdown was successful, the high landing weight and sink rate caused the left main gear to collapse, leading to a loss of directional control and a fatal collision with concrete bridge pillars.
