This caused moisture to freeze into a thin, transparent layer of ice on the upper wing surfaces. The crew and the Flying Station Engineer (FSE) performed a walk-around but concluded that the wings were merely wet. Because the Fokker 100 lacks leading-edge slats, its wing is extremely sensitive to even minute amounts of surface roughness, which can cause a premature stall and loss of roll control during rotation.

Final Conclusions

The primary cause was the loss of roll controllability due to wing icing. The flight crew and maintenance personnel lacked 'ice awareness' and failed to request de-icing despite meteorological conditions conducive to icing.
