A United Airlines Douglas DC-8 and a TWA Lockheed Super Constellation collided over Staten Island while navigating toward their respective New York airports. The United jet, suffering from a failed radio receiver, overshot its assigned holding pattern at high speed and entered the airspace assigned to the TWA flight. The TWA aircraft crashed onto Staten Island, while the United DC-8 continued for several miles before crashing into the Park Slope neighborhood of Brooklyn. Investigation Summary

The collision occurred at approximately 5,000 feet in heavy cloud cover and falling snow. United Flight 826 was inbound to Idlewild Airport (now JFK) and had been instructed to hold at the Preston intersection. However, one of the aircraft's two VHF Omni-directional Range (VOR) receivers was inoperative, making it difficult for the crew to identify the intersection precisely. Combined with a high approach speed, the DC-8 overshot the holding point by approximately 11 miles. TWA Flight 266 was under radar vectors for an approach to LaGuardia Airport when the United jet struck it from the side. The investigation highlighted the dangers of high-speed jet operations in a congested terminal area still largely reliant on procedural separation and manual navigation, especially during the poor visibility caused by the winter storm.

Final Conclusions

The Civil Aeronautics Board (CAB) determined the probable cause was United Flight 826 proceeding beyond its cleared limit and the confines of the airspace allocated to it. A contributing factor was the high speed of the DC-8 as it approached the Preston intersection, coupled with the failure of the crew to notify air traffic control of their inoperative navigation equipment, which would have necessitated different handling in the busy New York corridor.
