However, he became confused by the nature of the emergency and repeatedly asked First Officer Weiss for assistance. Weiss, despite being a senior captain acting as a first officer, did not respond to the captain's queries, instead focusing on radio communications with Air Traffic Control. This breakdown in Crew Resource Management (CRM) was exacerbated by an unwritten 'silent cockpit' rule at Midwest Express, which discouraged non-essential communication below 800 feet. During this silence, the pilots allowed the nose to pitch up, causing the airspeed to decay. The aircraft entered a stall and rolled sharply to the right. The NTSB also noted that the left engine may have suffered a power loss due to the ingestion of debris from the right engine or atmospheric disturbances caused by the stall, but the primary cause was the crew's failure to maintain control of the aircraft during a survivable mechanical emergency.
