Investigation Summary
The aircraft was operating a non-scheduled flight to Tampa and was approximately 117 pounds over its maximum allowable takeoff weight of 48,000 pounds. During engine start-up, witnesses noted smoke from the right engine, yet the crew proceeded with the flight. Immediately after liftoff, Newark tower controllers observed white smoke and alerted the crew. A Miami Airlines captain on the ground, mistakenly believing the smoke was caused by an overheated brake, advised the crew via the tower to extend the landing gear. This decision significantly increased aerodynamic drag. As the fire intensified, the right engine lost power and the aircraft began losing altitude while attempting to return to the airport. At an altitude of approximately 200 feet over Elizabeth, the aircraft stalled, entered a steep left bank, and struck a house and a brick storage building used by the Elizabeth Water Company before coming to rest in the Elizabeth River.
