A departing Iberia Boeing 727 collided with an Aviaco DC-9 that had inadvertently taxied onto the active runway in dense fog at Madrid-Barajas Airport. The DC-9 crew, disoriented by low visibility and inadequate taxiway markings, missed their intended turn and entered the runway just as the 727 reached V1 speed. The collision and subsequent fire destroyed both aircraft, killing all 42 people aboard the DC-9 and 51 people aboard the 727.

Investigation Summary

On the morning of December 7, 1983, Madrid-Barajas Airport was blanketed in thick fog with visibility reported around 300 meters. Iberia Flight 350, a Boeing 727, was cleared for takeoff on Runway 01. Simultaneously, Aviaco Flight 134, a DC-9, was instructed to taxi to the holding point for the same runway via the outer taxiway. Due to the lack of clear visual references and inadequate signage, the Aviaco crew became lost in the fog. Instead of following the taxiway to the threshold, they inadvertently turned onto the active runway. As the Iberia 727 accelerated through its takeoff roll and reached V1, the crew suddenly saw the DC-9 through the fog. The Iberia captain attempted to rotate early to clear the obstacle, but the aircraft's left wing and landing gear struck the DC-9. The impact triggered a massive fire that consumed both airframes.

Final Conclusions

The CIAIAC determined the primary cause was the unauthorized incursion of the DC-9 onto the active runway. This was attributed to the intense fog which prevented the crew from obtaining sufficient visual references to navigate the taxiway system.
