Investigation Summary
The accident originated from a landing gear indication problem during the final approach to Warsaw. The crew, suspecting a burnt-out bulb or fuse, elected to perform a go-around to allow the flight engineer time to verify the gear status. When Captain Lipowczan increased thrust to initiate the climb, the low-pressure turbine shaft of the number 2 engine failed. The resulting disintegration of the turbine disc released three large fragments.
