Investigation Summary
The accident occurred during the helicopter's eighth flight of the day. After departing the Downtown Manhattan Heliport at 14:58 EDT, the pilot flew a teardrop pattern south of the Statue of Liberty and then proceeded north along the east side of the Hudson River adjacent to Manhattan. At 15:08, the pilot performed a U-turn near the George Washington Bridge and began heading south along the New Jersey side of the river. ADS-B data shows the helicopter was maintaining an altitude between 625 and 675 feet. At 15:15, witnesses reported hearing loud noises similar to gunshots and seeing feathers in the air as the helicopter began to disintegrate. Surveillance video captured the fuselage yawing severely as the tail boom failed and the main rotor system separated. The fuselage fell upside-down into shallow water near the Newport neighborhood of Jersey City, while heavier components like the rotor and gearbox settled in deeper water. The NTSB recovered the main rotor system, transmission, and tail rotor by April 14, 2025. Laboratory analysis by the Smithsonian Institution identified remains of Canada geese, Brant geese, and a great black-backed gull on the rotor blades and horizontal stabilizer.
Final Conclusions
The investigation is ongoing. The NTSB released a preliminary report on May 7, 2025, and a factual docket containing over 2,000 pages of information on July 16, 2026. While a formal probable cause has not yet been issued, the NTSB confirmed that the helicopter struck multiple large birds (including Canada geese which can weigh over 10 pounds), causing the structural failure of the tail boom and rotor system. A final report is expected by the end of 2026.
