Investigation Summary
The approach to Yogyakarta was characterized by a total breakdown of Cockpit Resource Management (CRM) and a fixation on landing by the Captain. Although cleared for a visual approach, the Captain continued with an unplanned ILS approach to Runway 09 while the aircraft was significantly above the glide slope. To compensate, he initiated a steep descent that caused the airspeed to increase to 241 knots, far exceeding the 250-knot limit below 10,000 feet and the 141-knot target for landing. During the descent, the GPWS issued 15 'Sink Rate' and 'Pull Up' warnings, all of which were ignored. The First Officer requested a go-around and attempted to set the flaps to 15 degrees as requested by the Captain, but the flaps only extended to 5 degrees because the high airspeed triggered a load-relief mechanism to prevent structural damage. The First Officer did not inform the Captain of the flap configuration discrepancy. The aircraft touched down 240 meters beyond the nominal zone at 221 knots. The high energy of the landing caused the nose gear to fail, and the aircraft overran the 2,200-meter runway, coming to rest 252 meters beyond the threshold of the reciprocal Runway 27. The investigation also explored a fuel efficiency incentive program implemented on March 1, 2007, as a potential motive for the Captain's refusal to go around. The post-crash fire was exacerbated by the airport's inadequate rescue response, where fire hoses were punctured by vehicles driving over them and the airport fence blocked access to the crash site.
Final Conclusions
The Indonesian National Transportation Safety Committee (NTSC) concluded that the primary cause was the Pilot in Command's failure to follow stabilized approach criteria and his decision to continue the landing despite an excessive descent rate and airspeed. The NTSC also cited the First Officer's failure to take control of the aircraft when the Captain ignored repeated warnings, a failure attributed to inadequate training and a steep cockpit authority gradient. Contributing factors included the lack of a sufficient Runway End Safety Area (RESA) at Yogyakarta and the failure of the airport's emergency services to effectively coordinate the fire suppression effort. The NTSC released its final report on 22 October 2007.
