Investigation Summary
The mission was a low-level training flight designed to practice terrain-avoidance techniques for evading Soviet radar. Originally designed for high-altitude nuclear penetration, the B-52 fleet was being adapted for high-speed, low-altitude operations (approximately 500 feet AGL) to counter improved surface-to-air missile (SAM) threats. This operational shift placed immense stress on the airframe, particularly when flying through lee waves and mountain-induced turbulence. During the flight, the aircraft encountered severe buffeting. The investigation determined that the turbulence-induced stresses caused the stabilizer shaft to break after the rudder connection bolts failed. The vertical stabilizer was found 1.5 miles from the main impact site. This was the second tall-tail B-52 to suffer a fatal structural failure due to turbulence. The first occurred on June 23, 1959, when a B-52D (serial number 56-0591) out of Larson AFB suffered a horizontal stabilizer failure at low level. The Elephant Mountain incident, along with three similar vertical stabilizer failures within the following year, led Boeing to strengthen the rudder connection bolts across the entire B-52 fleet.
