Investigation Summary

The accident occurred in the heavily congested Kharkiv air traffic control sector. On the day of the crash, a novice controller was assigned to the busy sector under the supervision of a senior controller. The sector was further complicated by the presence of a high-priority flight carrying Soviet leader Leonid Brezhnev, which led to altitude restrictions for other aircraft. Flight 7880 (heading to Minsk) and Flight 7358 (heading to Kishinev) were placed on intersecting paths at the same altitude of 8,400 meters. The junior controller realized the conflict too late and issued a vague command intended for Flight 7880 to climb. However, the radio transmission was stepped over by another aircraft, and the crew of Flight 7880 never received the instruction. The two aircraft collided in clouds at a combined speed of approximately 1,000 km/h.

Final Conclusions

The investigation concluded that the primary cause was gross negligence by air traffic control. Specifically, the controllers failed to maintain safe separation, ignored conflict warnings on their displays, and issued a critical instruction that was not confirmed by the receiving crew. The junior controller and his supervisor were both sentenced to 15 years in a penal colony.
