On October 26, 1989, the Boeing 737-200 was scheduled for a short domestic flight to Taipei-Sungshan Airport. The crew, consisting of an experienced captain and a novice first officer, departed from Runway 03 at Hualien Airport. However, they mistakenly followed the Standard Instrument Departure (SID) intended for Runway 21. While the correct procedure for Runway 03 required a right turn toward the open sea, the Runway 21 procedure dictated a left turn. This error directed the aircraft straight into the Chiashan mountains. The aircraft struck the terrain at an elevation of approximately 7,000 feet (2,100 meters) roughly three minutes after takeoff, approximately 5.5 km north to 10 km northwest of the airport. The impact and subsequent fuel explosion destroyed the aircraft, leaving no survivors. Contributing factors included poor flight preparation, failure to follow the pre-takeoff checklist, and limited visibility due to night conditions.
